Aligned Lifestyle Assessment
If you complete the Aligned Lifestyle Assessment, the information you provide is used to deliver and help you understand your assessment results and may be used to provide relevant educational information, resources, or communications.
The assessment is provided for educational and informational purposes only. Assessment responses and results do not constitute a diagnosis, medical assessment, mental health assessment, or individualized medical advice.
The assessment is currently hosted using Tally, a third-party form provider. Tally processes and stores form responses on my behalf.

If you voluntarily subscribe to my email list or request a resource that includes email communications, your contact information may be stored and processed through Mailchimp, my email marketing service provider.
